Is this a real Pokemon card which is rare ("Diamond") a holo / holo reverse? Or is it false?
Yes Each set consists of Rare Holo Pokemon expansion of Rare Holo no, not uncommon and common cards Holo Non-Holo. In addition to this, there are also copies of Reverse Holo each card in the game (A bit like a parallel series). Each booster contains – a rare (perhaps Holo, perhaps not Holo) one Reverse Holo (may be a rare, uncommon or common), 3 and 5 Uncommon Common (From Diamond and Pearl). Hope this helps!
